Serve

Serve as plenary or limited guardian of the person and/or property, guardian advocate, client advocate, health care surrogate, health care proxy, durable power of attorney, representative payee, advocate and life long friend.

Vigorously

Vigorously defend the beneficiary’s rights and assist in the receipt of the following opportunities: education, habilitation, treatment and training which meet the standards fair wage for work performed, right to privacy, right to vote, adequate food and clothing, religious freedom and practices, freedom to marry and appropriate recreation and social opportunities.

Consult

Consult with the Beneficiary to see that they are happy and satisfied with their lives, including living, work and recreation situations.

Monitor

Monitor health needs of the Beneficiary and seek appropriate health care services utilizing available health care benefits.

Inspect

Inspect living conditions making sure they are clean, neat, and in good repair. Monitor food and choices given in residential situations and monitor shopping/nutritional choices in more independent situations. Working situations will be monitor periodically for basic health and safety conditions.

Maintain

Maintain contact with immediate supervisors of residential situations, supported living, employment coaches, support coordinators, non-residential support services, behavior specialists and all others in the circle of support.

Employment

Work with the Beneficiary when he/she needs assistance; help them select the best service providers, living situations, companions and employment situations available to them.

Entitlement

Seek out and obtain any government or other benefits to which the Beneficiary is entitled by law.

Support

Attend and give input at annual support plans or any other diagnostic and evaluation settings for the Beneficiary. Monitor the use of iBudget funds and budgets.

Beneficiaries

Monitor the Beneficiaries management of monies received from all sources.

Management

Monitor and ensure that all individual trust funds, wages, petty cash, checking and savings accounts are properly utilized for the benefit of the Beneficiary; to the extent Advocare that has control of those funds.

Feedback

Report quarterly and more frequently if needed, to any relative or family friend designated by the Member.

This report will include but not be limited to:
• Residential setting and services.
• Medical situation and services received during the quarter
• Day program or work situation
• Financial review
• Recreation and leisure activities
• All other issues of importance in the lives of the Beneficiary
